In the following statements, the ‘I’ signifies each member in the registrant's family/group listed below on whose behalf the registrant (you) is signing.

1. I hereby agree with ASDFW, its Council of Trustees and Event Organizing Committee, that I will not bring about any suit of action of law against ASDFW, its Council of Trustees and Event Organizing Committee, not institute, prosecute, or in any way aid the prosecution of any claim, demand, action, or cause of action from damages, costs, loss of service, expenses or compensation for or on account of any and all known and unknown, foreseen and unforeseen bodily and personal injuries, damage to property, and the consequences thereof, resulting from my participation in ASDFW Events.

2. I understand that any property damage, temporary or permanent, which I cause, is solely my responsibility, and I am prepared to be held accountable for all such damages.

3. I give unrestricted rights to use my image, name, voice and likeness for all non-commercial written, audio and or visual presentations. I understand that the written, audio and/or visual presentation may be used in print, broadcast and online promotion to promote event and/or to share with other attendee(s).

4. I have read, understood, and agreed to accept and abide by the conditions listed above. Once this contract has been signed, it can and will be upheld in a court of law if deemed necessary.
